Our Practice’s Policies And Procedures
Welcome to Internal Medicine Wellbeing PLLC (“Internal Medicine Wellbeing,” “Practice,” “us,” or “we”)! Our practice specializes in Internal Medicine with a focus on the patient’s overall well-being. We offer personalized medical evaluations and medical care, weight loss solutions, and IV drips (“Services”). Please contact us if you have questions about your options and eligibility for receiving Services. Thank you for entrusting us to care for your health and well-being. Before starting or continuing treatment, we require all patients to agree to and sign these policies and procedures. They set forth the terms of our relationship while you receive these Services.
Please take time to read this and raise any questions you may have prior to signing below. We may update these policies and procedures (“Policies and Procedures”) at our discretion, and we will provide you with a copy of any updates. By signing below, you acknowledge and agree to be bound by these updates.
Privacy, Medical Records, And Mandated Reporting
- Notice of Privacy Practices. We comply with all state and federal medical privacy laws, including the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the Health Information Technology for Economic and Clinical Health Act (HITECH). These laws require us to protect the privacy and security of your health records and personal information. We have implemented privacy policies and procedures to ensure the Practice’s compliance with these requirements. This information is summarized in the Notice of Privacy Practices. Please ask if you have questions about how we protect your privacy.
- Medical Records. We maintain records about your treatment. If you want a copy of your records, or if you want us to send your records to another provider, please ask us for a medical records request form. Please note that, in some instances, we may charge reasonable and cost-based copying, postage, scanning, or digital storage device fees.
- Child Abuse. New York law requires that we report all child abuse or maltreatment to local law enforcement or the New York Office of Children and Family Services Child Protective Services (CPS) division. We must call CPS if we have reasonable cause to believe that a child who is known to us in our professional capacity may be abused or neglected. It is our policy to notify you prior to contacting CPS if, in our professional judgment, it is reasonable to do so.
- Elder Abuse. New York law also allows us to report the abuse of elderly or disabled adults to the Bureau of Adult Protective Services. This may include physical, sexual, financial, or psychological abuse, neglect, or exploitation. It is our policy to notify you prior to making a report if, in our professional judgment, it is reasonable to do so.
- Social Media Policy. The Practice and its Service providers will not accept friend or contact requests from current or former patients on their personal social networking accounts. Adding patients or contacts on social networking sites can compromise your confidentiality and our respective privacy. It may also blur the boundaries of our professional relationship.
Payment And Cancellation Policies
- Payment. We are a cash-only practice and ask that you pay at the time of service. We do not accept Medicare, Medicaid, or private third-party payors. You understand that you are wholly responsible and liable for payment of all charges assessed for professional Services rendered. If you decide to file your own claims for out-of-network reimbursement, your insurance may not cover these Services, or it may consider them to be subject to lower out-of-network benefits. We do not guarantee that your insurance will cover the Services. Your insurance company may not pay some fees, and these fees will be your responsibility to pay.
- Good Faith Estimate. You are entitled to receive a Good Faith Estimate of the expected charges for your services. The Practice will provide you with a Good Faith Estimate before your scheduled services or upon request.
- Cancellations. We have reserved your appointment time specifically for you. If you need to cancel or reschedule your appointment, please contact us during business hours and at least 24 hours prior to your scheduled appointment to avoid paying a cancellation fee of $100. For Monday appointments, you must cancel by 12:00 p.m. on the previous Friday so we can offer that time to another patient. If you do not show up for your appointment or cancel with less than an hour’s notice, we will charge you a no-show fee of $100.
Communication With Us
- Communications. By providing us with your personal contact information or by initiating communication with us by unsecured email or text message, you authorize us to call, leave voicemails, and respond to your text messages. We will only contact you for non-marketing purposes, including appointment reminders, billing, invoicing updates, and treatment questions.
- Use of unencrypted methods of communication. You understand that communicating with us by unencrypted emails and via text or social media messages may be unsecure. This also means that any of your protected health information (“PHI”) that is transmitted in this way, including information about your appointments, diagnosis, progress, and other individually identifiable information, may be unsecure. If you communicate via text, email, or social media, please limit the content to general information (such as scheduling or asking for a time to talk via phone). When communicating protected health information to us, we ask that patients make an appointment or use secure messaging systems. Please be aware of privacy risks when using electronic means of communication.
Safety And Security
- Safety and Security Policy. For mobile visits, we ask that you acknowledge and agree that there is adequate space available at your location for the services to be rendered and that it’s generally a clean environment. The Practice clinician, in their sole discretion, may choose to terminate the appointment or Services if the clinician determines the environment is unsafe or otherwise inadequate to render Services.
- Privacy. For mobile visits, we ask that you provide a space with as much privacy as possible. You acknowledge that the Practice cannot control who is in the space and you acknowledge and accept all privacy risks associated with receiving services in the home or at a location of your choosing.
- COVID-19 and other diseases. We ask that you notify the clinician if anyone in your home has tested positive for COVID-19 or another illness in the two weeks prior to your appointment. Because COVID-19 is an ongoing public health concern, we may require you to take certain protocols to limit the spread of the virus. Any COVID-19-related protocols will be explained to you before the administering Services, and you agree to follow these protocols.
Thank you for choosing Internal Medicine Wellbeing! We look forward to taking care of you.
